EHR: CPOE for Medical Laboratory & Radiology Orders

EHR Stage 2: Core Measure 1

Overview

A certain percentage of medical laboratory orders and radiology orders must be entered by a licensed provider. This is called Computerized Provider Order Entry (CPOE). To count as CPOE in Open Dental, when the order is created, a Provider must be Logged-On.

Exclusions Available? Yes.  Providers qualify for an exclusion if they order less than 100 of each type of order during the reporting period. 

Medical lab orders: These orders are usually for blood work.

Radiology orders: We do not know how your state will interpret this measure. Some states allow dental providers to claim an exclusion since they don't usually 'order' outside radiology orders. Other states, such as New Jersey, require that providers manually enter orders for bitewings, panos, etc. Contact your state to find out what is required and get it in writing. We recommend that providers who cannot claim an exclusion set time aside to enter at least 30% of orders. We realize this is an onerous task that is outside of a dental workflow. In addition, we have found a LOINC radiology code for bitewings, but not for other common dental x-rays. We recommend using it. The resulting measure percentage will likely be 100% since order entry increases the denominator and entry by provider increases the numerator (1 order entered by 1 provider equals 100%).

Meanwhile, Open Dental is working on a simpler solution to help incorporate this task into workflow.

Calculations

CPOE_LabOrdersOnly: In Measure Reports, the percentage is calculated using the following information:

  • Denominator: The number of medical laboratory orders for an 'ordering provider', created by a provider, with a date/time start during the reporting period.
  • Numerator:  Orders in the denominator entered entered by a provider.

CPOE_RadiologyOrdersOnly: In Measure Reports, the percentage is calculated using the following information:

  • Denominator: The number of radiology orders (orders with a 'RAD' LOINC code) for an 'ordering provider', created by a provider, with a date/time start during the reporting period.
  • Numerator:  Orders in the denominator entered by a provider.
