Overview

Record changes to height and weight (all ages) and blood pressure (age 3 and over) for patients seen by the provider during the reporting period.

Calculations

In the Measures Report, there are three calculation options for vital signs.

Vital Signs:  Calculates height, weight (all ages) and blood pressure (ages 3 and over). Use if not applying for any exclusions.

  • Denominator:  All unique patients (age 3 and over for blood pressure) with at least one completed procedure by the Provider during the reporting period.
  • Numerator:  Patients in the denominator with height, weight, and blood pressure recorded.

Vital SignsBMIOnly:  Only calculates height and weight entries. Use if applying for an exclusion from blood pressure.

  • Denominator:  All unique patients with at least one completed procedure by the Provider during the reporting period.
  • Numerator: Patients in the denominator with height and weight recorded.

Vital SignsBPOnly:  Only calculates blood pressure entries. Use if applying for an exclusion from height and weight.

  • Denominator:  All unique patients age 3 and over with at least one completed procedure by the Provider during the reporting period.
  • Numerator: Patients in the denominator with blood pressure recorded.

CQMs

Three Clinical Quality Measures depend on vital sign entries and interventions:

  • #69:  Preventive Care and Screening: BMI Screening and Follow-Up
    There are two calculations based on age. For patients 18 years of age or older, if BMI is outside of normal parameters an intervention should be documented.
  • #155:  Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents
    There are multiple calculations based on age (3 - 17). Some calculations only depend on the entry of vital signs, others also require documentation of an intervention.
  • #165 Controlling High Blood Pressure
    Measures patients 18-85 who have a hypertension diagnosis (problem) and an adequate blood pressure measurement during the reporting period.
