Medical History 

From the main Toolbar, click Forms.  See the Patient Forms page for information on how to use this feature. As explained on that page, medical history forms can be filled out on paper and then scanned, they can be filled out on the Kiosk, or they can be filled out as Web Forms.  The data from medical history forms does not get imported into the database.
