Problems In the Main Menu, select Setup | Problems.
You can add, delete, and reorder problems from here. If a patient is already linked to a problem, you can safely hide the problem so that it won't show up for other patients. It will still continue to show for patients already using it. To attach a problem to a patient, go to that patient's Medical page. Click Add Problem (or Add ICD9 if using the EHR). Once added, then you can double click to edit the problem.
There are three possible statuses: Active, Resolved, and Inactive. Resolved and inactive are largely synonymous, the only difference being that a resolved problem is acute and will never return. For example, a fractured hip or an infection would become resolved, while diabetes or angina might become inactive for a while.
